Enrollment Information

Legacy Family Daycare is accepting enrollments for children 6 weeks to 2 years of age.

If you would like to enroll your child(ren) in our daycare, please complete the form listed below and we will contact you within 24 hours to discuss your specific childcare needs and schedule an in person or virtual interview, as well as a tour of our daycare facility. We look forward to answering any questions you may have.

In the event that our daycare is full to capacity, we do have a waiting list to accomodate families at a later date as spaces become available.

Your child’s enrollment is complete and your daycare spot reserved when all required paperwork is completed and turned in including:

  • Enrollment Contract

  • State of CT Enrollment Application

  • Immunization Records (current and signed by doctor)

    • Including Flu Shot

  • Emergency Medical Form

  • Emergency Contact Form

  • Authorization for Administration of Medications

Note: Immunizations must be current and should be updated annually.
